​Certified Nursing Assistant (CNA) & Home Health Aide (HHA)
Nursing assistants and home health aides provide personal care, home making, and companionship to our clients under the supervision of a registered nurse (RN) or a licensed therapist (PT). 
Personal Care Services:
  • Assist with Activities of Daily Living (ADL)
  • Assist with bathing
  • Prepare, serve meals
  • ​Light house keeping
  • Respite Care
  • Companionship
  • Assist with ambulation, transfers
  • Assist with self administration of medications​
  • Companionship & Respite: Having a person by our client's side while the primary caregiver is at work, running errands, on vacation, or simply when some extra help is needed (short-term and long-term).
  • Personal Care: Assist with bathing, grooming, medication reminders, meal preparations, etc.
  • Homemaking: Light Housekeeping, making / changing beds, dusting, washing dishes, cleaning kitchen counter tops and appliances, taking out trash, etc. ​
  • Escort: Attend doctor's appointments together with our Nurses (RN, LPN) or Certified Nursing Assistants (CNA/HHA). Our staff will help our clients understand the visit and report back to the primary caregiver, or simply accompany our clients for some extra support. -Private Duty Only

Our goal is to maximize our client's functional independence at home and educate the client and caregivers with tools to implement on a daily basis. Our therapists evaluate each client on an individual basis in order to establish a plan of care in accordance with the physician's orders and the client's specific needs.
Physical Therapy
  • Physical therapy services assist in restoring and maintaining maximum musculature function. Our therapists are specialized in treating a multitude of diagnosis, from hip fractures to sprains and unsteady gait. Through the treatment of these conditions, our therapists assist clients in decreasing their fall risk restoring their confidence in living independently within their home environment.
Occupational Therapy
  • ​​Occupational therapy strives to improve the client's level of independence in their activities of daily living such as bathing, grooming, and personal care.
Speech Therapy
  • ​​Speech therapy assists in improving swallowing disorders and the communicative activities of daily living.

Medical Social Worker
Facilitate long term planning to promote independence and provide opportunities to take advantage of a variety of community resources designed to assist the client with their personal concerns and help alleviate their troubles.
